NDIS Support Coordination (Level 2 & 3)

Our Support Coordinators work alongside NDIS participants and their families to make the most of every plan. We help you understand your funding, connect with the right providers, and build the skills and confidence to direct your own supports.

What is Support Coordination?

Support Coordination is NDIS funding that helps you understand your plan and put it to work. Your Support Coordinator connects you with the right providers, sets up your services and makes sure everything works together, so you can focus on your goals, not the paperwork.

When you work with Authentic Care & Support Services, your coordinator takes the time to understand your goals, your circumstances and the people around you. We become part of your team and you become part of ours.

  • Understanding and implementing the funded supports in your plan.
  • Connecting you with providers, negotiating services and pricing, and making sure service agreements and bookings are in place.
  • Linking you with community, mainstream and other government services.
  • Tracking your budget and preparing for your NDIS plan reviews.
  • Practical help when circumstances change or something isn’t working.

Who is it for?

Support Coordination is for participants who have Coordination of Supports or Specialist Support Coordination funded in their plan. Level 2 helps you build the skills to coordinate your own supports over time. Level 3 is for more complex situations and is delivered by a practitioner with specialist skills. If you’re not sure what your plan includes, we’ll help you check.

What’s the difference between Level 2 and Level 3 Support Coordination?

Level 2 (Coordination of Supports) helps you understand and implement your plan, connect with providers and build your ability to coordinate supports yourself over time.

Level 3 (Specialist Support Coordination) is for participants whose situations are more complex. It is delivered by a practitioner with specialist skills, who helps manage challenges in the support environment and ensures consistent service delivery. You can read more at the NDIS website.

I already have a Support Coordinator, how do I change to Authentic Care & Support Services?

It’s always best to let your current Support Coordinator know you’re looking to change providers. Then contact us at [email protected] and request Support Coordination. Our intake team will guide you through the move.

How long would I work with a Support Coordinator for?

There’s no set time limit. Our coordinators always work to build your independence, and can support you through plan reviews and into future plans. As you gain confidence navigating the NDIS, you may find you need less coordination over time. That’s a good outcome.

Where would we meet?

Wherever works for you: at home, in the community, at one of our offices, or by phone and video. Our Support Coordinators are flexible and fit around your routine.

How is Support Coordination funded?

Support Coordination is funded from the Capacity Building budget of your NDIS plan, under Coordination of Supports (Level 2) or Specialist Support Coordination (Level 3). It does not come out of your Core supports.
